"He knew because he helped arrange it," Rebecca said.
Claire stood beside the emergency department window, looking at Dominic through the glass. "That's an accusation. What can you prove?"
"That Elias called my office at nine this morning. That Dominic hadn't authorized him to cancel. And that Elias predicted he'd be unable to make decisions for a long time. Keep the clinic records safe. I'm coming."
The call ended. Claire turned to Leah. "Rebecca's bringing the original appointment instructions."
"The hospital is preserving the medical files," Leah said. "The records officer has the emails from Kessler's clinic."
Inside the treatment room, a physician explained the scan to Dominic. The small object lay close to irritated tissue around his lower spine. The doctors could not tell from the images alone exactly what it was or whether it had caused the infection. They wanted a spine team at a larger hospital to assess him as soon as he was stable enough to move.
Claire had spent years missing the certainty of an operating room. Tonight she was grateful the attending did not pretend to know more than the images could show.
Dominic caught her eye through the window and gestured for her to come in.
"My brother?" he asked.
"Rebecca is on her way. Marcus is here. Elias hasn't come to the hospital."
"That wasn't the question."
"Your brother canceled your meeting with Rebecca this morning. He told her you wouldn't be making decisions for a long time."
Dominic looked at the ceiling. "He has spent six months telling me I can't shut down the old routes."
"Then give Rebecca the records."
"They implicate me too."
"I assumed they might."
He turned toward her. She could see him deciding whether her calmness was indifference. "You didn't ask what I did."
"You're my patient tonight. The rest is a conversation you'll have with your lawyer and investigators."
"Would you have stayed if you'd known?"
Claire pulled the chair closer to the bed. "I knew your name when I walked in."
Before he could respond, a hospital administrator entered with a man in a gray suit. The man introduced himself as counsel for Kessler's clinic. He wanted to discuss the photographs Claire had taken and her "professional standing." Dominic watched Claire go still.
"My standing has nothing to do with the images," she said.
"You represented yourself to the family as a surgeon."
"I told them I was the agency nurse."
"Your license history may complicate that claim."
Claire had trained as a registered nurse before medical school. When St. Alden pushed her from surgery, that older credential and an agency willing to employ her had kept her working. The clinic's lawyer was counting on the awkward history sounding suspicious to people who did not know it.
Leah came in behind him. "The patient's treating team has the images. This conversation can happen elsewhere."

"They were obtained at a private residence," the lawyer said.
"They document the patient's condition," Leah replied. "Please take any legal request through hospital counsel."
The administrator led him away. Claire found she had dug her nails into her palm.
Three years earlier, she had stood before the medical board with an entire box of altered surgery schedules. She had expected questions about the patients. Instead, the hospital's lawyers asked why she had accessed the records, why a resident once corrected her medication order, why her marriage had failed. She had answered every question. None brought the patients back into the room.
"He's going to circulate my file," she told Leah.
"Then document this visit too. Who he is, what he said, who heard it."
Claire wrote it down while the words were fresh. It felt small compared with what Elias could do. It was still something she could control.
She called the agency supervisor as well. The supervisor confirmed that Claire had been assigned as a nurse, that her credential was current, and that the agency had received no request from Dominic to remove her. Claire asked her to preserve the assignment notes and the original medication chart. Then she emailed herself the exact wording of the lawyer's accusation, with Leah and the administrator named as witnesses. Her hands steadied as the messages left her phone.
Rebecca arrived with a paper copy of Dominic's instructions and a laptop full of company documents. She did not open the latter in the hospital corridor. Instead, she showed Claire the appointment notice Dominic had signed two weeks earlier. Friday's meeting concerned records from Varela Freight and several subcontractors. Elias had no authority to cancel it.
"I want a copy of the message he sent," Dominic said when Rebecca entered his room.
"You will have it. For now, tell me exactly what you authorize."
Dominic took a breath. "Preserve the accounts. If I'm unable to speak tomorrow, release them to the investigators we discussed."
Rebecca wrote down his words and asked the attending to note the time and Dominic's ability to answer questions. Claire watched from the doorway. The process was deliberate; nobody treated a frightened man's whispered instruction as a magic document.
Later, Rebecca mentioned Kessler's clinic by its corporate name. Claire recognized it.
"Say that again," she asked.
"Briar Clinical Partners."
Claire's stomach tightened. Briar had appeared in the billing records she uncovered at St. Alden Medical Center. It had received consulting fees while St. Alden's administrators approved unnecessary procedures. Claire had flagged the name in her report, but no one had interviewed its principals.
"I need my old files," she said.
Leah was watching her. "Claire, Briar's name on an old invoice won't make tonight your old case again."
"It's already my case. Kessler knew my history before I knew his name. I need to know whether he learned it from a medical network or from someone who wanted me frightened."
"What kind of connection?" Rebecca asked.
"I don't know yet. Briar was paid by the hospital that ruined my career. I'll find out what it was paid for."
Claire phoned the former hospital records clerk who had kept a lawful copy of her original complaint. By dawn, a redacted invoice was in her inbox. Beneath Briar's name was a second signature: Simon Kessler.
